The combivent inhalation aerosol is a combination of ipratropium bromide (anticholinergic bronchodilator) and albuterol sulfate (beta 2-adrenergic bronchodilator). The combivent inhalation aerosol maximizes the response to treatment in patients with chronic obstructive pulmonary disease (COPD) by reducing the bronchospasm through two different mechanisms, anticholinergic and sympathomimetic.

Details

Ipratropium bromide is an anticholinergic (parasympatholytic) agent which inhibits vagally-mediated reflexes by showing the antagonizing action of acetylcholine, the transmitting agent released at neuromuscular junctions in the lung. Anticholinergic prevent the increase in intracellular concentration of Ca++ caused by interaction of acetylcholine with muscarinic receptors on bronchial smooth muscle.

Albuterol sulfate has a preferential effects on beta2-adrenergic receptors compared with isoproterenol. Activation of beta2-adrenergic receptors on airway smooth muscle leads to activation of adenylyl cyclase and increase in intracellular concentration of cyclic AMP.The increased concentration of cyclic AMP leads to the activation of protein kinase A, due to which the phosphorylation ofmyocin gets inhibited and intracellular ionic calcium concentration gets decreased, resulting in relaxation.

Dosage and method of administration:

The dose of this aerosol is two inhalations four times a day.

The aerosol is supplied as a metered-dose inhaler with a white mouthpiece that has clear, colorless sleeve and an orange protective cap. This aerosol canister is to be used only with Combivent Inhalation Aerosol mouthpiece and not with other mouthpieces. This mouthpiece should not be used with other aerosol medications

Uses of combivent inhalor:  

Combivent inhaler is used in patients having chronic obstructive pulmonary disease (COPD) on regular aerosol bronchodilator who continue to have evidence of bronchospasm (airway narrowing) and require second bronchodilator.

Drug interactions of combivent inhalor:

The various drug interactions shown by this medication are:

  • This medication shows an additive interaction with anticholinergic medications.
  • When taken with other sympathomimetic agents, this medication can result in increased risk of adverse cardiovascular effects.
  • The coo administration of beta receptor blocking agents and albuterol inhibit the effect of each other.
  • The hypokalemia effect of diuretics can be worsened when coadministered with beta-agonists.
  • Combivent inhalation aerosol should be administered with extreme caution in the patients that are being treated with monoamine oxidase inhibitors.
  • With furosemide, the albuterol can increase the risk of developing hypokalemia, or low blood potassium.

Contraindications of combivent inhalor:

The drug is strictly contraindicated in the following cases:

  • In the patients having history of hypersensitivity to soya lecithin or related food products like soybean and peanut.
  • In patients showing hypersensitivity to other components of drug products.

Precautions of combivent inhalor:

The precautionary measures that are to be taken into consideration while the usage of the drug is:

  • Combivent inhalational aerosol should be used with caution in the patients with narrow angle glaucoma, prostatic hyperplasia or bladder-neck obstruction.
  • The drugs containing amines should be used with caution in patients with convulsive disorders, hyperthyroidism or diabetes mellitus and in patients who are responsive to sympathomimetic amines.
  • The drug should be used with precaution in hepatic or renal disease.

Side effects of combivent inhalor:

The side effects of the drug include:

Edema, fatigue, hypertension,dizziness,nervousness,tremor,tachycardia dysphonia,insomnia,diarrhea,dry mouth, vomiting, palpitation,angina,taste perversion, pruritus, urticarial, angioedema, lips and face, laryngospasm etc.

Overdose:

Overdosing of the medication should not be done. In case if overdosing symptoms occurs, immediately visit to the nearest hospital.

Missed dose:

If the dose is missed, it should be taken as soon as get remember. But if the time of next dosage schedule has been followed, the missed dose is skipped and the next regular dosing schedule is followed.

Storage conditions:

The medication should be stored properly at temperature 250C. It should be avoided from excessive humidity. Keep out of reach of children and pets.
